Records review for a coordinated evaluation
If prior labs, imaging, or operative reports exist, sharing them in advance makes an evaluation more useful. Records review is educational and does not constitute a second-opinion diagnosis.
What may be useful to share
- Recent labs — fasting glucose, HbA1c, lipids, liver function, TSH, vitamin panels
- Height, weight history, and current medications (including GLP-1 dosing history)
- Operative reports if you have had prior bariatric or abdominal surgery
- Recent imaging reports (upper GI, CT, MRI) if available
- Sleep study results if performed
- List of comorbid conditions and specialists you currently see
How records are handled
Records shared with a coordinator are treated as confidential clinical information and are only reviewed by authorized clinical staff for the purpose of a coordinated evaluation. See our privacy policy for detail.
How to send records
Complete the request form on the home page or contact the coordinator via WhatsApp. The coordinator will confirm which items are relevant and provide a secure channel to send them. Do not send records through unsecured public forms or email attachments to unknown addresses.
